I based it on the Tibetan Buddhist refuge formula. (cf. https://www.lamayeshe.com/article/preliminary-practice-refuge) My adaptation allows for a wider context than the specific Tibetan Buddhist one.

I read through the entire linked article. What resonated most were The Four Immeasurable Thoughts. Both they and your adaptation are simple enough for me to appreciate and practice. Thanks, Ben.
